Sagittal Plane Blockage: Understanding the Domino Effect

The blockage in the sagittal plane can be local (pelvis, spine) or transmitted from the foot. Functional hallux limitus prevents the terminal tilt of the first ray and disrupts balance.

The body compensates by an anterior tilt of the pelvis and a projection of the trunk, generating constraints on the lower limb and the lower back.

Normal walking: an economical movement

Physiological walking is similar to an inverted pendulum, with no concentric contraction during the stance phase. Energy is returned by segmental mechanics.

When the tipping point is lost, energy saving disappears, muscle chains become over-activated and fatigue sets in more quickly.

Functional hallux limitus is a bit like the Columbus egg of orthopedics.

Biomechanical consequences on posture and the lumbar spine

The lack of sufficient dorsiflexion at the first ray disrupts the 'rockers' of walking. The knee remains flexed, the hip struggles to extend, and the trunk vectors compensate.

These adaptations can contribute to low back pain and dynamic balance disorders.

Observable clinical signs and identification

On the mat, a lateral sweep and a posterolateral heel strike may be visible. At the end of the support, pronation persists while supination should be established.

Joint analysis of support and kinematics guides the treatment plan.

Loss of the reference tipping point causes the upper body to fall forward.

Plantar supports and a graduated approach

Specific arch supports can restore the tipping point and reduce compensations. They are part of a rehabilitation program focused on coordination and posture.

This strategy aims to restore more efficient walking and reduce associated pain.

Perspective for the patient with low back pain

Back pain is not inevitable. A foot and gait assessment may reveal an unknown contributing factor.

Correcting the blockage upstream improves general mechanics and contributes to the lasting reduction of symptoms.
