Understanding functional hallux limitus in a few key points
The term "functional hallux limitus" describes a sliding blockage of the long flexor of the big toe (FHL) in the hindfoot. This condition prevents normal extension of the big toe at the end of support and alters the gait.
This pathology affects both the biomechanics of the foot and overall balance. Indeed, an alteration in propulsion disrupts stability and can lead to distant pain.
Simplified anatomy and mechanism of blockage
The FHL muscle runs from the posterior compartment of the leg, passes behind the talus in a fibrous-bony tunnel, and then ends on the big toe. Blockage occurs at the musculotendinous junction during passage through this tunnel, particularly when the ankle is dorsiflexed.
This mechanism defeats the winch effect of the forefoot and creates compensations throughout the lower limb.
Functional hallux limitus is a clinical entity characterized by the absence of sliding of the long flexor tendon of the big toe.
Recognize the signs and make the diagnosis
Observation of gait and the three-step stretch test are usually sufficient to guide the diagnosis. A dynamic podiatric impression can demonstrate a lack of support under the first ray and a late tilt into pronation.
Simple exercises already allow us to assess the progress: when the subtalar joint is released and the tendon slides again, extension of the hallux becomes possible again in the ankle dorsiflexion position.
Treatments: from conservative to minimally invasive
The first step combines a release maneuver, targeted stretching, balance work, and, if necessary, dynamic arch supports to guide footing. The goal is to resynchronize gait and improve balance.
When symptoms persist, endoscopic surgery through two short incisions frees the passage of the tendon in the retrotalar tunnel and restores dorsiflexion of the hallux at the end of support.
This blockage creates a cascade of side effects on the joints and disrupts our static and dynamic balance.
Practical advice for the patient
A regular exercise program, attention to step sequence, and gradual adaptation of physical activity promote improvement. Coordination with the therapist allows for adjusting goals and monitoring progress.
